Imagine contracting HIV and becoming too weak to work, losing your job, your home and having to choose between food and medication.

Where would you turn to rebuild your life? Strengthen your body? Lift your spirits?

Vital Bridges – at the forefront of helping men, women and children impacted by HIV and AIDS build healthier lives for over twenty years.

In 1988, an HIV diagnosis for most was a death sentence. Today, HIV is still incurable but those who have the disease – whether rich or poor – can lead fruitful and fulfilling lives with the right medications and resources.

Vital Bridges to Merge with

Heartland Health Outreach

We are excited to let you know that Vital Bridges will merge into Heartland Health Outreach effective July 1, 2011. The Boards of Directors of the organizations voted in late May to approve the merger after several months of investigation. Vital Bridges will become the Vital Bridges Center on Chronic Care, a division of Heartland Health Outreach.

All current client services and sites will be sustained. Program staff will also remain in the roles they held previously. In the last twelve months, we made a commitment to better serving our clients by increasing our capacity to address the health aspects of HIV and other chronic diseases. Joining Heartland will help us to do this even more successfully, as they have a large, professional medical and behavioral health staff. Both organizations are committed to developing and implementing a chronic care model that integrates health care and the social services.

As always the commitment of our steadfast donors and volunteers is critical to our continued ability to provide food, nutrition counseling, housing, case management and educational services to the nearly 2,000 men, women and children living with HIV/AIDS that we have the privilege to serve each year.

We look forward to the next chapter in our organizational journey. Thank you very much for your investment in our clients and mission.
